Review of patients for admission criteria and continued stay criteria. Formulate discharge plan, interview patients and family members for discharge needs. Collaborate with physicians, nursing and ancillary staff as well as payers. Enter medical reviews into QRS system. This position does not provide direct patient care however may provide case management services to all age groups as needed. Minimum Education: California RN License preferred. Experience: Case Management experience preferred. Recent acute hospital experience preferred. Knowledge of Interqual Criteria, Medicare, MediCal and Insurance regulations preferred. Good written and verbal communication skills. Computer skills preferred. Full time is 40 hrs week, 8 hr shifts. |
